CONTRACT OF AGREEMENT
FOR SERVICES PROVIDED BY LAILIA’S PET BNB
FEES ARE PER PET, NOT PER ROOM.
Non-refundable deposit, per room required on reservation. (Deposit will be deducted from total bill at time of departure). 48 hour cancellation notice requested so we may open your reservation to another pet(s).
We must have your current up-to-date immunization records from your vet on file. Lailia’s Pet BNB requires all the required immunizations regulated by Texas State Laws, including bordatella (Kennel Cough) immunization. Lailia’s Pet BNB can administer Bordatella at the kennel for a fee of $20.00 per pet. This immunization needs to be given at least 72 hours prior to pets’ stay, please make arrangements accordingly. Agreement to terms and conditions releases Lailia’s Pet BNB from any liability due to illness of any kind.
Signed Contract of Agreement gives parent permission to Lailia’s Pet BNB to act on their behalf in the case of injury or ailment. Parent is agreeing to have Lailia’s Pet BNB act on their behalf and seek medical advice/treatment when necessary in their absence. An attempt will be made to contact the parent of the pet(s) at the contact numbers made available to Lailia’s Pet BNB. Parent fully understands and agrees that if they cannot be reached or the situation does not allow for time to reach parent, Lailia’s Pet BNB will take care of the medical needs of the pet(s) and the parents will be responsible for reimbursement of vet expenses/medications and follow up care of the pet(s). Parents are responsible for full reimbursement of expenses incurred during their absence to care for their pet(s). Lailia’s Pet BNB is not responsible for injury, illness or death to any pet(s) during, or following their stay at the kennel.
If your pet(s) injure another client or an employee, parent is responsible for any expenses incurred as a result of injury. If quarantine is warranted, as regulated by Texas State Law, parents will be responsible for the additional fees of quarantine.
Lailia’s Pet BNB requires all paperwork to be completed and signed before pet(s) can be left at kennel. Paperwork and immunization records on file should be updated each year.
If your pet(s) have fleas, Lailia’s Pet BNB will dip your pet(s) for a $20.00. Please notify us if you think your pet(s) has a flea problem as consideration toward all other clients staying with us.
Upon request Lailia’s Pet BNB will bathe/groom your pet(s). Charges will vary according to size, breed and service provided. If your pet(s) require bathing during their stay, they will be bathed at parents’ expense.
Any special needs or instructions for your pet(s) must be given at time of drop off. Parent is responsible for letting Lailia’s Pet BNB know of any special conditions your pet(s) may be sensitive to or any special considerations your pet(s) may need.
Lailia’s Pet BNB will administer medications, if full instructions are provided with medications. If pet(s) requires extensive administration of medications there may be an additional charge. Please only bring the medication for the days the pet will be staying with us plus a 2 day emergency supply.
Parent needs to bring/provide food for your pet(s) during their stay. Pets will get an upset tummy if their diet is changed during their stay. Please only bring the amount of food your pet will consume during their stay.
Parents do not need to bring personal items for their pet(s). We will provide everything your pet(s) need during their stay. If you decide your pet(s) can't be without an item, please make sure all items are labeled with pet’s name. Lailia’s Pet BNB can not be responsible for lost or damaged items. All pets must be brought with a leash.
CHECK OUT TIME IS BEFORE 12:00 NOON. IF YOU CHECK OUT BEFORE NOON YOU ARE ONLY CHARGED A $5.00 CLEAN UP FEE PER ROOM. IF PET(S) ARE PICKED UP AFTER 12:00 NOON, PARENT WILL BE CHARGED FOR A FULL DAY’S STAY.
There is someone on the premises at all times during business hours. Your pet(s) will have outdoor playtime, unless requested not to.. Pet(s) are let out alone, or with their family members unless requested not to. We have 4 play yards and each dog receives 3-4 outdoor playtimes per day. Since we live on the property there is someone here at almost any time. Please be considerate and make arrangements to pick up/request to pick up your pet(s) during business hours only.
SALON/HOTEL/DAYCARE GUESTS
Lailia’s Pet BNB requires current vaccinations.
Proof of annual rabies and DHLPP+C, which has been administered by a veterinarian,
is required. Bordatella is required and must be current within 6-12 months (whatever
your veterinarian specifies). I understand that Lailia’s Pet BNB will
either verify vaccinations with my veterinarian verbally, or I can bring written
proof of vaccinations. The only exceptions are for older pet(s) whose veterinarians
decide they cannot have the vaccinations or for pet(s) using holistic vaccination
methods. I understand Lailia’s Pet BNB is not responsible for any illness
my pet(s) may incur while at the facility due to their not having state mandated
vaccinations.
I hereby declare that my pet(s) is current with regard to Lailia’s Pet
BNB policy on vaccinations. Although Lailia’s Pet BNB prefers not to do
this, in the case of emergency and with my consent, Lailia or Ricky Ferrell
will administer the intranasal dose of Bordatella at a charge of $20.00 per
dose per dog.
I understand that Bordatella can cause an allergic reaction and/or create a
false positive and/or produce the actual illness in a pet, any and all of which
will require treatment by a veterinarian. I understand that I am responsible
for veterinarian charges should any of these situations occur after my pet is
vaccinated.
I understand that my pet(s) should have a rabies vaccination. If my pet(s) do
not have current proof of rabies vaccination, I understand that I will be responsible
for any bills incurred for a quarantine of my pet(s) if my pet(s) bites a staff
member of Lailia’s Pet BNB or another pet(s) visiting the facility during
their visit. I understand if my pet(s) harms another pet(s) or employee of Lailia's
Pet BNB, I will be responsible for any bills incurred as a result of the aggression.
I understand that pets, like humans, are susceptible to various strains of viruses
and bacteria and can become ill even under the best conditions. I understand
that because there are many types of bacteria and viruses, vaccinations are
not 100% effective. I understand that much depends on the dog’s immune
system, exposure to other pets, age stress and nutritional levels. I understand
that Lailia’s Pet BNB takes prudent measures to ensure the good health
of all their guests and that I am responsible for veterinarian charges should
my pet(s) become ill during or after their stay at Lailia’s Pet BNB. I
also understand if my pet(s) develop a condition that goes undetected by Lailia’s
Pet BNB, I will not hold Lailia’s Pet BNB responsible for the condition/injury
or result of, while pet(s) in the care of Lailia’s Pet BNB.
CONTRACT OF AGREEMENT
I have been made aware that any item(s) that I bring for
my pet(s) could possibly become lost/damaged during my pet(s) stay at Lailia’s
Pet BNB. Lailia’s Pet BNB requires items be labeled. I understand items
may still be lost/ damaged. Items found after my departure will be returned
provided courier expenses are reimbursed. I will not hold Lailia’s Pet
BNB responsible for replacement of any items lost/damaged during my pet(s) stay.
FLEAS AND TICKS
I understand that Lailia’s Pet BNB uses an outside
company monthly to provide pest extermination services inside and outside the
facility. I understand that Lailia’s Pet BNB makes every reasonable attempt
to guard against flea/tick infestation, and I will not hold Lailia’s Pet
BNB responsible if my pet(s) go home with fleas or ticks. I understand that
I have the option of requesting at my cost, a flea shampoo/dip if I choose before
my pet(s) depart Lailia’s Pet BNB. I understand if fleas/ticks are detected
on my pet(s) during their stay, a shampoo or dip may be provided at the cost
of the parent.
IMMUNIZATION WAIVER
I am aware and understand that my pet(s) is not up-to-date
on all of their shots. I understand that Lailia’s Pet BNB is not responsible,
if my pet(s) becomes ill or deceases. Lailia’s Pet BNB is not responsible
for illness or injury resulting from delayed immunization and any medical bills/expenses
incurred because of this will be fully reimbursed to Lailia’s Pet BNB
at time of pick up. If pet(s) decease, payment will be made to Lailia’s
Pet BNB immediately for their length of stay and any expenses incurred as a
result of their condition.
REIMBURSEMENT OF EXPENSES FOR VETERINARIAN CHARGES
I have toured Lailia’s Pet BNB and have thoroughly
researched the services and the environment in which temporarily custodial services
are provided for my pet(s). I feel comfortable that Lailia’s Pet BNB makes
every attempt to provide a safe environment for my pet(s). I understand that
accidents can/do happen (ie. Dogs do play and socialize with each other in neighboring
rooms, and although rare, a dog fight can result in a bite or a cut; dogs jump
up and down and wag their tail, which can result in a sprain, cut or broken
limb; etc). I have had all my questions answered to my satisfaction. I understand
I may visit the facility during business hours to visit with my pet(s). Customer
references will be provided by Lailia’s Pet BNB upon request.
I approve of Lailia’s Pet BNB to make a sound judgment on seeking medical
advise or treatment for my pet(s). Any cost incurred to obtain this advice or
treatment will be parent responsibility.
I agree that although there are other options to Lailia’s Pet BNB and
the services they provide, I have made the choice to leave my pet(s) at Lailia’s
Pet BNB for salon/hotel/daycare services.
I understand that Lailia’s Pet BNB will not be responsible for the loss
of my pet(s) should they find a way to escape the premises.
I understand that Lailia’s Pet BNB may feel medical advice/treatment is
necessary. An attempt will be made to contact me, if time is available. An attempt
will be made to contact primary veterinarian of the pet, but I understand advice/treatment
may be provided by the veterinarian chosen by Lailia’s Pet BNB if needed.
I understand that I am responsible for all charges incurred for medical advice/treatment,
transportation, etc to provide necessary treatment to your pet(s).
I understand in lieu of my primary veterinarian, Lailia’s Pet BNB may
use a veterinarian of her choice. I understand Lailia’s Pet BNB uses veterinarians
local to Rockwall that provide reputable service to the community of Rockwall
where Lailia’s Pet BNB does business.
I will not hold Lailia’s Pet BNB responsible for veterinarian charges
for the treatment of my pet(s) if they begin to show signs/symptoms of illness,
or if they sustain an injury while receiving salon/hotel/daycare services at
Lailia’s Pet BNB.
UNSPAYED AN UN-NEUTERED PET(S)
I understand that Lailia’s Pet BNB prefers to provide
services to pet(s) who have been spayed/neutered. I also understand that Lailia’s
Pet BNB takes reasonable precautions to keep unsprayed females who are “in-heat”
away from un-neutered males; therefore I will not hold Lailia’s Pet BNB
responsible for pregnancies or other problems which may arise as a result of
my pet(s) not being spayed/neutered.
MISCELLANEOUS ITEMS
I understand Lailia’s Pet BNB is not responsible for lost or damaged items.
I understand Lailia’s Pet BNB has designated business hours for pick up and drop off and will not expect pick up or drop off service outside of these hours.
Pick up/drop off services are provided by Lailia’s Pet BNB within a designated area and for an extra charge.
I understand the services provided by Lailia’s Pet BNB and agree to pay billed charges upon completion of services.
I understand Lailia’s Pet BNB makes every attempt to keep my pet(s) clean, however during play time or in kennel may become dirty (muddy, spilling food, bladder/bowel accident). These may require a bath which will be provided at cost to parent without prior consent.
Lailia’s Pet BNB must be notified within one business day after completion of services if I am not satisfied with salon services. I may return my pet(s) within two business days to correct clipping or scissoring services.
I understand Lailia’s Pet BNB has the right to refuse service to a pet(s) who is aggressive, unmanageable, or disruptive to the other clients staying at Lailia’s Pet BNB.
I understand any medical expenses to an employee of Lailia’s Pet BNB as a result of an action by their pet(s) will be fully reimbursed by parent of the pet(s).
I understand Lailia’s Pet BNB will not release my pet(s) to any other person except to those who have been designated by the owner as responsible party upon departure.
In my absence, I hereby give my permission to Lailia’s Pet BNB to act as my agent to seek medical advice/treatment for the below mentioned pets at parent expense.
Although I prefer my primary veterinarian, I give Lailia’s Pet BNB permission to use the vet of their choice if necessary for medical advice/treatment of my pet(s) at parent expense.
